Who needs to complete this form?
All families new to Bangor Township Schools must complete this form for each student - even if your student will NOT ride the bus. This ensures our records are accurate for every student in the district.
Any student who will ride the bus at any time during the 2026–2027 school year - every day, certain days, or only occasionally - must have a completed form on file.
Please submit a separate form for each student. Forms are due by AUGUST 14, 2026. If your student needs transportation to/from school as their primary transportation, this form must be completed or your student will not be able to ride the bus for the 2026–2027 school year.
⚠️ Forms submitted after August 14 may result in a delay before bus service begins. Your student's routing cannot be guaranteed for the first day of school. Please submit as early as possible.
